Make a LEAD Referral
Thank you for your interest in making a LEAD (Let Everyone Advance with Dignity) referral. Providing the information on the next page allows us to quickly assess an individual's eligibility. If you are unsure if an individual is LEAD eligible we still encourage you to submit this form and a LEAD project manager will follow up with you.
Our goal is to engage individuals referred and approved with wrap-around case management and services that create long term behavior change, ultimately resulting in a reduction in unlawful behavior. We do this by connecting them to case managers, with whom they set individual goals and work to obtain those goals through accessing relevant services. These services include, but are not limited to, accessing legal income, addressing medical needs, addressing court obligations, obtaining identification and documents, chemical dependency treatment, mental health care, housing, and job training and placement.
